Passport Applications
FEES EFFECTIVE: January 28, 2019
All applicants must appear in person.
The following items MUST be submitted with your passport application.
- EVIDENCE OF CITIZENSHIP: One of the following documents must be submitted with your application and it will be returned to you in a separate mailing from your newly issued passport.
- BIRTH CERTIFICATE: a CERTIFIED copy of birth certificate issued by the COUNTY or STATE. It must show the birth record was filed within one year of birth and full name (s) of the parent (s). It must have the Registrar’s signature and the raised, embossed, or multi-colored seal of the Registrar’s office. A notarized copy or a hospital birth certificate is not acceptable.
- NATURALIZATION CERTIFICATE: a person who claims US citizenship by naturalization must submit the original Certificate of Naturalization with the application.
- EXPIRED PASSPORT: this is acceptable if you are 16 years of age or older.
- PHOTO: (2”x2”) full front view of face, taken within the last six months, and must be in color. PASSPORT PHOTOS ARE AVAILABLE at the Chickasaw County Recorder’s Office for a fee of $10.00.
- PERSONAL IDENTIFICATION: a photocopy of the front and back side of a valid driver’s license (not driver’s permit). Anyone under 16 must have parent(s) along with the parent(s)’s ID for parental consent and signature.
- FEE: each passport should be paid by two checks or money orders as indicated below:
- PASSPORT BOOK:
- $110 payable to U.S. Department of State – Valid 10 Years ($80 if under 16 years of age – valid 5 years. )
- $35 payable to Chickasaw County Auditor-this payment can be cash.
- PASSPORT CARD:
- $30 payable to U.S. Department of State – Valid 10 Years ($15 if under 16 years of age – valid 5 years)
- $35 payable to Chickasaw County Auditor-this payment can be cash.
- Expedited fees (processing in 10 – 15 business days) Additional $60 per passport.
- PASSPORT BOOK:
- COMPLETED APPLICATION – Do not sign until requested to do so.
Passport applications may be obtained in the Auditor’s or Recorder’s office or download the form from: https://travel.state.gov
